Therapy Without a Family Doctor in Ontario: Routes That Work

You can start free therapy in Ontario without a family doctor. Ontario Health says you can register yourself for Ontario Structured Psychotherapy, ConnexOntario says no referral is needed, and Health811 and Good2Talk state no referral requirement. The OHIP-insured routes are different. The Schedule pays a psychiatric consultation as a consultation only when it follows a written request from a physician or nurse practitioner, and it does not say whether a patient can be seen without one, so ask the psychiatrist's office. If a request is needed, it does not have to come from a family doctor.
Routes that do not need a family doctor
What each program says about doctors and health cards, taken from its own pages or from the Government of Ontario. "Not stated" means the program's pages do not say.

What each OSP network says
Ontario Structured Psychotherapy is run by regional networks, and they say different things about family doctors:
- York Region and South Simcoe: states that not having a family doctor is not a barrier, and that a provider's referral is not required.
- Mississauga, Brampton and Halton: states you do not need a family doctor or an Ontario health card.
- Thunder Bay and Northwestern Ontario: its FAQ states you do not need a primary care provider or family doctor.
- Southwestern Ontario and Niagara: says you do not need a health card or to be a Canadian citizen to be referred, and does not address family doctors.
- The other networks accept self-referral through an online form or phone but do not say whether you need a family doctor: Toronto, Central North, Eastern Ontario, Ontario Shores and the North East.
The insured route: what needs a doctor
The Schedule of Benefits defines a consultation as an assessment that follows a written request from a referring physician or nurse practitioner. If the written request is missing, the amount payable for the consultation is reduced to a lesser assessment fee. The Schedule does not say whether a patient can see a psychiatrist without that request, so ask the psychiatrist's office before you book.
What the Schedule does not state is a referral rule for psychotherapy itself, in either direction. So whether a physician who provides psychotherapy takes patients directly is decided by the individual practice: the Schedule states no rule either way, and we did not find one elsewhere. The Schedule says the written request can come from a physician or a nurse practitioner. It does not have to come from a family doctor, so another physician or a nurse practitioner can make it.
Getting a family doctor or nurse practitioner
- Health Care Connect. The Government of Ontario says it finds a family doctor, nurse practitioner or primary care team that is accepting new patients in your community. You register online or by calling 811, and you can register your whole family. It requires a valid Ontario health card number, and the province says joining does not guarantee a clinician will be found immediately.
- Health811. Call 811 (TTY 711) at any hour. Its tools also include a searchable directory of health care services, doctors, nurse practitioners and specialists.
- Community Health Centres. The province lists them, and names people without health insurance among the groups they focus on. Its page does not say whether centres offer counselling, so ask the centre.
- A walk-in clinic. BounceBack's own FAQ suggests taking its form to a walk-in clinic provider who is willing to take short-term clinical responsibility while you are in the program.
Do you need a health card?
Four sources say a card is not required: OSP West and CarePoint Health for referral, and ConnexOntario and Health811 for contacting them. CMHA York says there is no charge regardless of OHIP status. For most other routes the pages do not say. Health Care Connect is the exception: it requires a valid number. If you do not have a card, ask the program directly before you rely on it.
