Free Therapy in Ontario: The Main Routes That Cost You Nothing

Free therapy in Ontario comes from four different places: the province's funded program, Ontario Structured Psychotherapy (adults 18 and over); BounceBack guided self-help (age 15 and over); services for students and young people; and psychotherapy from a physician, which OHIP insures.
They are all free to you, but they are open to different people and each has its own way in. Psychotherapy from a physician is insured through OHIP. Ontario Health describes Ontario Structured Psychotherapy as free and publicly funded, and CMHA York says it is not billed through OHIP. Several of the others accept self-referral.
Free is not one thing
Three different mechanisms make care free in Ontario, and mixing them up is the most common mistake in online advice.
- Insured by OHIP. OHIP pays physicians for medically necessary services, so therapy delivered by a physician and billed to OHIP costs you nothing. It is a separate route with its own rules; see how OHIP-covered therapy works.
- Publicly funded programs. Ontario Structured Psychotherapy is described by Ontario Health as free and publicly funded, and delivered through regional networks. CMHA York says it is not billed through OHIP.
- Helplines and services. ConnexOntario gives information and referrals, Health811 gives advice from a nurse, and Good2Talk offers counselling and crisis support to students. They are free, and each does something different, so read its row below.

Community Health Centres are another place to start, and the Government of Ontario's page names people without health insurance among the groups they focus on. That page does not state what they cost or whether centres offer counselling, so ask the centre.
Match the route to your situation
- Adult with depression or anxiety who wants therapy. Start with Ontario Structured Psychotherapy. The regional guides below say how to refer yourself in each area.
- Aged 15 to 17. BounceBack takes teens through an intake form. One Stop Talk and Kids Help Phone are also listed by the Government of Ontario.
- Post-secondary student. Good2Talk is available at any hour, and students aged 18 and over can also refer themselves to Ontario Structured Psychotherapy.
- Want longer-term therapy. Ontario Structured Psychotherapy is short-term. Psychotherapy from a physician is insured, and we found no annual per-patient maximum in the Schedule of Benefits, though finding a physician who provides it is the hard part. See medical psychotherapy.
- No family doctor. Several free routes need none. See therapy without a family doctor.
- Want to talk to someone now. Call or text 9-8-8 for suicide crisis support at any hour. In an emergency call 911.
What free does not include
The programs say plainly what they do not do, and it matters more than the headline that they are free:
- Ontario Structured Psychotherapy is short-term, is not a crisis service, and is not suitable for everyone. Networks that publish criteria exclude people at high risk of suicide, people whose main focus is self-harm or an eating disorder, and people with uncontrolled or recent symptoms of mania or psychosis, with the wording varying by network. It does not provide medication management.
- BounceBack coaches are not clinicians and do not provide counselling or diagnosis.
- ConnexOntario does not provide counselling, psychotherapy or clinical treatment.
- The Health811 nurse does not give a diagnosis or prescribe medication.

What we do not publish
We do not publish an average waiting time, a number of sessions you can expect, or availability. Several programs publish a figure for part of the process, such as the time to a first contact or the length of a course, and others publish none. Each regional guide gives exactly what that network states, and says so when it states nothing. If timing matters, start a second route in parallel.

Is there free therapy in Ontario?
Yes, in several forms. Ontario Structured Psychotherapy provides free, publicly funded cognitive behavioural therapy for adults with depression or anxiety-related concerns. BounceBack provides free guided self-help with phone coaching from age 15. Post-secondary students can use Good2Talk, and physician-delivered psychotherapy is insured by OHIP. How do I get free therapy in Ontario without a doctor?
Ontario Health says you can register yourself for Ontario Structured Psychotherapy. Two networks state outright that a doctor's referral is not required, and others accept self-referral. BounceBack allows self-referral but asks for your primary care provider's details, ConnexOntario says no referral is needed, and Health811 and Good2Talk state no referral requirement. Is there free therapy for students in Ontario?
Good2Talk is a free, confidential service for post-secondary students in Ontario, available around the clock by phone or text. It provides professional counselling, crisis support and referrals. Students aged 18 and over can also use Ontario Structured Psychotherapy, and students aged 15 to 17 can use BounceBack.
Is ConnexOntario therapy?
No. ConnexOntario is a free helpline that gives information and referrals for mental health, addiction and problem gambling services by phone, text, chat or email at any hour. It states that it does not provide counselling, psychotherapy or clinical treatment.
